To book with us, please follow the below steps:

1 -Book and appointment: Call 01925 597655, WhatsApp message or book online.

2 - Complete the Grey sections of your D4 form before arrival at the appointment, D4 forms can be collected from local Post Office or downloaded using this link. A D4 form can also be supplied in clinic

Please bring your latest glasses prescription if glasses are worn for driving or for daily to your appointment and an up to date list of current prescription only medication from your GP.

Please arrive 10 mins early for your appointment for visual acuity and blood pressure check.

Bring your in date Driving licence or passport for Photo ID.
